Adult Foster Care Services can be provided to individuals who have:
- Developmental Disabilities
- Aging Concerns
- Mental Illness
- Medical Conditions
Eligibility includes:
- Individuals over the age of 16.
- Individuals who need assistance with activities of daily living.
- Individuals who need cueing/supervision and/or assistance with Activities of daily living including bathing, dressing, walking, transferring, etc.
- The caregiver must reside with the individual.

What is Adult Foster Care (AFC)?
Adult Foster Care is a state-funded program. It allows individuals above the age of 16 with physical and/or intellectual disabilities who require assistance with activities of daily living. The program promotes individuals to stay in their homes and the community with their caregivers.
The individual may require assistance or supervision with ADLs.
ADLs include Bathing, Dressing, Toileting, Transferring, Walking, and Eating.
